第二篇：請假條的格式
請假條的格式
由於某種原因你不能去上學，或不能參加某項活動，就應向老師請假。
怎樣寫請假條呢？
請假條一般應包括以下幾個部分：
（1）稱呼。要項格寫，並在後面加冒號。
（2）正文。要另起一行，空兩格。主要寫明請假的原因和請多長時間假。
（3）請假人姓名和日期。寫在正文的左下方。

我因要去參加少年宮的大合唱彩排，今天下午的班隊活動不能參加，特此請假。
